Prior to your first visit with Dr. Meehan, we ask that you complete the Patient Information/Health History Form.
You may download this form to your computer and print it out. This form can also be mailed to you before your scheduled appointment.
Please complete the form and bring it with you to your first appointment along with any insurance card or information.
Initial Exam and Consultation
- Your first visit with our office is complimentary and will include an initial exam and consultation.
- You will spend approximately 30 minutes with Dr. Meehan to evaluate and discuss your treatment needs.
- Dr. Meehan will review your medical and dental history and perform a non-invasive clinical exam.
- Dr. Meehan will address your questions and concerns.
- If treatment is indicated, Dr. Meehan will discuss the treatment plan with you and work together with your general dentist to ensure the highest level of oral health.
- If treatment is indicated, the cost of treatment will be determined as well as the next step to beginning treatment.
- Appointments to begin the treatment process will be scheduled.
Diagnostic Records Appointment
- Records are taken prior to beginning orthodontic treatment and provide information not available from the clinical exam.
- The appointment will last between 30 to 45 minutes.
- Two x-rays, facial and dental photographs, and impressions of your teeth will be taken at this appointment.
- These records are used to customize your treatment plan and track your progress throughout the course of your treatment.
Beginning Treatment
- Your starting appointment to put braces on or to insert an appliance usually takes from 1 to 2 hours.
- This appointment is scheduled earlier in the day in order to ensure the proper amount of time and care in beginning your treatment.
- We will be more than happy to provide an excuse for school or work if needed.
- Your financial contract and insurance claim form will be created and processed at this appointment.
- After completing your "starting" appointment you will then see us again periodically for an adjustment which will take approximately 15 minutes.
